Acute Progressive External Ophthalmoplegia Mimicking Acute Ophthalmoplegia without Ataxia

Abstract
Acute ophthalmoparesis (AO) is one of the anti-GQ1b antibody syndrome featured by acute postinfectious ophthalmoplegia without ataxia and elevated anti GQ1b titers. We present a 63 years-old woman with acute onset of bilateral opthalmoparesis without ataxia and normal deep tendon reflex. Serum antibody titers against GQ1b were not elevated and no other etiology for ophthalmoparesis was found despite extensive patient evaluation. Intravenous immunoglobulin was administered, and the patient gradually improved over subsequent months. This case is mimicking AO, but anti GQ1b profile was not matched.
